Welcome to the Chronic Care Education Project
Chronic Condition: A condition, such as asthma, type II diabetes, or heart disease, that continues indefinitely. Chronic conditions cannot be cured, but symptoms can be controlled with proper treatment and lifestyle changes, so that individuals with chronic conditions can maintain their quality of life. Maintaining a healthy lifestyle is one of the best ways to both control an existing chronic condition and prevent chronic conditions from developing.
The Chronic Care Education Project was designed to provide teachers with resources to educate their students about chronic conditions; to help students recognize their risk factors for chronic conditions; and to enable them to put in place healthy lifestyle behaviors that may prevent them from developing a chronic illness.
